签证申请表及在线填表确认页：请登录 http://cova.mfa.gov.cn 在线填写签证申请表。申请表填妥后须打印出确认页及完整表格，并在纸质确认页上签名。
请务必在上述网站如实、完整、清楚地填写电子签证申请表；打印后不得手动更改。
致客户（填表协助服务）
如在线填写过程中出现照片审核不通过，可继续填写其余内容并“保存”。请勿确认或导出申请表。
随后请通过电话或电子邮件联系我们的客服，提供：服务号码、申请编号和护照号码。我们将为您核对表格；经您确认后，由我们直接修改并导出表格，通过电子邮件发送给您。您只需打印并手写签名即可，避免重复在线填写。
Online Visa Application & Printing
Visa application form & confirmation page: please visit http://cova.mfa.gov.cn to complete the e‑form. After completion, print the confirmation page and the full form, then sign the paper confirmation page.
Please ensure the e‑form is completed truthfully, fully and legibly on the above website. No handwritten changes are allowed after printing.
For Customers (Form Assistance)
If your photo fails the online check, continue other sections and click “Save”. Do not confirm or export the form.
Then contact our customer service by phone or email with your service number, application ID, and passport number. We will review your form and, after your confirmation, update and export it for you and email it back. You can simply print and hand‑sign it to avoid duplicating the online entry.
