符合标准的照片 2 张 —— 2寸正面免冠 6 个月内光面、白底纸质近照 4 张。其中 1 张粘贴在申请表上。
照片光盘 —— 电子护照采用数码照片，强烈建议同时提供一份刻有同版数码照片的 CD 或 DVD 光盘（恕不接受移动硬盘或其他移动存储介质）。
原护照复印件 ——（如有 或身份证明复印带照片资料页（如有）。 如无护照复印件，须提供以下一种证明申请人中国国籍和身份的材料的复印件：户口簿、身份证或
出生公证等。
护照遗失/毁损声明 —— 请点击这里 下载、打印并签署。
在美及其他国家 合法居留证明原件及复印件 —— 如绿卡 、工作、学习签证、 I 797 表、 I 20 表等。

Incorrectly submitted documents may result in additional expenses and significant delays, as they will be returned by the embassy or consulate.
